enrollment forms all packaged into one PDF for ease of use.  You will need
Adobe Acrobat Reader  to open the files.  It's free, so don't worry!  If you have
problems with the PDF files please e-mail dwain1771@gmail.com with the subject
"LDDDC PDF" and explain the problem and someone will get back to you ASAP.


Please note that on 1/21/09 our enrollment forms have been updated.  Please
open these in Adobe Only!  If your dog is currently enrolled, the forms have not
changed from the originals.  Soon we will have an enrollment online without the
hassle of printing!
